MSHSAA Pre-participation Documentation - required by all athletes each year:  Pre-Participation Documentation form

MSHSAA Athletic Physical Form - All players must have a physical on file prior to the first day of practice for each sport.  This physical is valid for two years:  MSHSAA Athletic Physical Form

ATHLETIC INSURANCE

Parents can purchase optional student athletic insurance coverage through  K & K Insurance.  Several options are available.
